When should I add a Phase Change to Progress Monitoring?
To document an adjustment to the student’s intervention or instructional approach.
- When the student’s Progress Monitoring results indicate that the student isn’t responding
- When the approach being used to support the student has been changed. This could be due to staffing or schedule changes or as a result of a recent IEP meeting or addendum.

Stagnant Performance: The student’s performance has remained stable but not at proficiency level
Declining Performance:
The student’s performance has consistently declined
The student’s performance has consistently declined
Staffing or Scheduling Changes
Changes to the intervention or instructional approach NOT based on the student’s performance
- Changes in intervention or support schedule
- Increased time or frequency
- Instructional environment (small group size)
- Materials used by the student
- Changes in staff or delivery of instruction
